(dba OriginMeds and referred to as “OriginMeds,” “we”, “us”, or “our”) recognizes the importance of confidentiality and privacy regarding your personal information and your right to privacy. This Privacy Policy (“Privacy Policy”) applies to all information collected through our website, in email, text, and other electronic communications sent through or in connection with the website and/or any related services, sales, marketing, or events (we refer to them collectively in this Privacy Policy as the “Services”).  Information that is protected health information may be subject to additional protections, and you may have additional rights regarding such information, pursuant to the federal Health Insurance Portability and Accountability Act of 1996 and its implementing regulations (“HIPAA”), and similar state laws, as applicable.  OriginMeds is HIPAA compliant.

This Privacy Policy should be read in conjunction with the Terms and Conditions of Use for the website of which it forms part (the “Terms”). This Privacy Policy DOES NOT apply to information that We collect offline or any other websites not described in this Privacy Policy. or that you provide to or is collected by any third party.

1. ORIGINMEDS SERVICES

OriginMeds does not provide any medical services.  OriginMeds provides a platform for you to access a health care provider contracted with OriginMeds Inc.The content of this website is for informational purposes only and does not constitute professional medical advice, diagnosis, treatment, or recommendations of any kind, and is not intended for use in the diagnosis of disease or other conditions, or in the cure, mitigation, treatment, or prevention of disease.  The health and wellness resources made available through our Services are not a substitute for direct in-person health care services in all cases. The decision to focus on diagnosis, treatment recommendations, or both, rests with you and the health care provider. You understand that by coordinating and in certain cases consulting with OriginMeds or an affiliate health care provider through the Services, you are not entering into a physician-patient relationship with OriginMeds itself.  Depending on the website without discussing with your medical practitioner is solely at your own risk.

2. INFORMATION COLLECTED

We collect personal information including personal health information (“PHI”) that you voluntarily provide to us when using our website, when you express an interest in obtaining information about us or our products and services, when you participate in activities on the website, or when you otherwise contact us.   • To send administrative information to you or to send you the product, service, and new feature information and/or information about changes to our terms, conditions, and policies.
  • To process your payment if you purchase any products or services.

Prior to posting a testimonial, we will obtain your consent to use your name and testimonial.  If you wish to update or delete your testimonial, please contact us and be sure to include your name, testimonial location, and contact information.

  • Vendors, Consultants, and Other Third-Party Service Providers.  At times, we need to disclose your information to persons or organizations outside OriginMeds who assist us with our payment/billing and health care operations. We require these business associates and their subcontractors to appropriately safeguard your information.
  • We may allow selected third parties to use tracking technology on the website, which will enable them to collect data about how you interact with the website over time.   Unless described in this Privacy Policy, we do not share, sell, rent, or trade any of your information with third parties for their promotional purposes.
  • Business Transfers.  We may share or transfer your information in connection with, or during negotiations of, any merger, divesture, restructuring, reorganization, dissolution, or other sale or transfer of some or all of our assets, whether as a going concern or as part of bankruptcy, liquidation, or similar proceeding, in which personal information held by us about our users is among the assets transferred.
  • Third-Party Advertisers.  We may use third-party advertising companies to serve ads when you visit the website.  These companies may use information about your visits to our website and other websites that are contained in web cookies and other tracking technologies in order to provide advertisements about products and services of interest to you.

5. HOW LONG DO WE KEEP YOUR INFORMATION?

When we have no ongoing legitimate business need to process your personal information, we will either delete or anonymize it, or, if this is not possible, then we will securely store your personal information and isolate it from any further processing until deletion is possible.

7. WHAT ARE YOUR PRIVACY RIGHTS?

If you would at any time like to review or change the information in your account or terminate your account, you can contact us at help@originmeds.com. Upon your request to terminate your account, we will deactivate or delete your account and information from our active databases.  However, some information may be retained in our files to prevent fraud, troubleshoot problems, assist with any investigations, enforce our Terms and Conditions of Use, and/or comply with legal requirements.

You can unsubscribe from our marketing email list at any time by clicking on the unsubscribe link in the emails that we send or by contacting us using the details provided below.  You will then be removed from the marketing email list. Please note, however, that we will still need to send you service-related emails that are necessary for the administration and use of your account.  To otherwise opt-out, you can contact us at help@originmeds.com.

If you are under 18 years of age, reside in California, and have a registered account on our website, you have the right to request the removal of unwanted data that you publicly post on the website.  To request removal of such data, please contact us using the contact information provided, and include the email address associated with your account and a statement that you reside in California.  We will make sure the data is not publicly displayed on the website, but please be aware that the data may not be completely or comprehensively removed from our systems.

8. WHAT ARE MY HIPAA RIGHTS?

OriginMeds operates in accordance with the federal Health Insurance Portability and Accountability Act of 1996 and its implementing regulations (“HIPAA”), and similar state laws, as applicable.  

  • We are required by law to maintain the privacy and security of your protected health information. We will let you know promptly if a breach occurs that may have compromised the privacy or security of your information.
  • We must follow the duties and privacy practices described in this policy.
  • We will not use or share your information other than as described within this policy unless you tell us we can in writing. If you tell us we can, you may change your mind at any time. Let us know in writing if you change your mind.
  • You can access and inspect paper or electronic copies of certain PHI that we maintain about you. You may readily access much of your health information without charge by requesting it through privacy@originmeds.com. In line with set fees under federal and state law, we may charge you for a copy of your medical records.
  • You can request amendments, or changes, to certain PHI that we maintain about you that you think may be incorrect or incomplete. All requests for changes must be in writing, signed by you or your representative, and state the reasons for the request. If we decide to make an amendment, we may also notify others who have copies of the information about the change. Note that even if we accept your request, we may not delete any information already documented in your medical record. You can make such requests by contacting support@originmeds.com.
  • In accordance with applicable law, you can ask for an accounting of certain disclosures made by us of your PHI. This request must be in writing and signed by you or your representative. This does not include disclosures made for purposes of treatment, payment, or health care operations or for certain other limited exceptions. An accounting will include disclosures made in the six years prior to the date of a request.
  • You can request restrictions on certain of our uses and disclosures of your PHI for treatment, payment, or health care operations. We are not required to agree but will attempt to accommodate reasonable requests when appropriate.
  • You can request a restriction on certain disclosures of your PHI to your health plan. We are only required to honor such requests when services subject to the request are paid in full. Such requests must be made in writing and identify the services to which the restriction will apply.
  • You can request that we communicate with you through alternative means or at alternative locations, and we will accommodate reasonable requests. You must request such confidential communication in writing to each department you would like to accommodate the request
  • We are required to notify you in writing of any breach of your unsecured PHI without unreasonable delay and no later than 60 days after we discover the breach.

If you believe your privacy rights have been violated, you can file a complaint with privacy@originmeds.com. You can also file a complaint with the Secretary of the U.S. Department of Health and Human Services in Washington D.C. A complaint must be made in writing and will not in any way affect the quality of care we provide you.

9. DO WE MAKE UPDATES TO THIS PRIVACY POLICY?

We may update this Privacy Policy from time to time.  The updated version will be indicated by an updated “Revised” date and the updated version will be effective as soon as it is accessible.  If we make material changes to this Privacy Policy, we may notify you either by prominently posting a notice of such changes or by directly sending you a notification.  We encourage you to review this Privacy Policy frequently to be informed of how we are protecting your information.
